I am thinking of retrofitting the BSA on my car and wanted to know if anyone has installed one on a W212, W207 or W204.
Apart from the obvious (mirrors and 2 radars), do I need a new wiring harness or can it be plugged into the existing one for the parking assistant?
Anything else?

The blind spot sensors are connected to the CAN-BUS radar network (full configuration: central Distronic sensor + 2 front side sensors and 2 rear side sensors + radar control unit).
There is an active BSA system (I think it is only available with Distronic and all 4 side sensors, and a passive system with only the two rear sensors).
Remember that sensors, especially those that were before the pre-facelift, fail after 5 years due to water ingress. There is nothing that could prevent this. Luckily, since 2024 the sensors have become reproducible and cost only about 800 euros each :)
I advise against retrofitting the BSA!

I was thinking about two rear sensors since I have a pre-facelift 2010 model. Otherwise I think it will be very difficult as I don't have the required ESP module. Also, as you said, the pre-facelift version had only the main sensor in the middle for Distronic, and the side sensors were missing.
Do you have a diagram or part number to check prices, availability etc.?

I found this one from W218, is it compatible?

The pre-facelift 2010 model with Distronic and active BSA system has two side sensors at the front. I think only the passive system has only two sensors at the back.
Without part numbers and your VIN code, nothing can be said. If it is compatible, then it is only a small part of the whole system. You will need a new mirror glass, a new mirror wiring harness, possibly a body wiring harness, possibly a door module, possibly a different ECU, possibly a new front SAM, possibly a new rear SAM.
I strongly recommend against installing this system. Buy a car with the system.
